Chapter Information
The ACMA Maryland Chapter, established in 2003, represents Nurses, Social Workers, Physician Advisors, Nurse Practitioners, Community Health Workers, Case Management Assistants and other professionals who work in case management and transitions of care and who reside and/or work in Maryland, Delaware, DC or West Virginia.
ACMA National and Maryland Chapter membership allows you to better connect with case management professionals, gain access to resources in Maryland and surrounding states, while still ensuring you have access to the innovative resources and research available through ACMA's nationwide network of 8,000+ case management and transitions of care professionals.

The Chapter of Excellence program defines outstanding chapter performance by outlining the standards of practice for optimal chapter operation. Chapters achieve these awards by demonstrating performance in the key areas of chapter operations and attaining specific growth and development benchmarks.
The Maryland Chapter received Chapter of Excellence achievement in the following years: 2024, 2023, 2022, 2019, 2018, 2017, and 2015. And the chapter was the proud winner of the ACMA National Award for 2019 Highest Attended ACMA State Chapter Conference in the Nation!!!

Speaker: Leanna Foglia, LCSW-C, Clinical Director Resource Group Counseling and Education Center
Abstract: People who necessitate a hospitalization, planned or unplanned, may experience stress, anxiety, and/or depression. The medical process, including diagnosis and proposed treatment, can be challenging for any person to understand. With a thorough assessment, the case manager can examine and plan for ways psychological, social, and emotional needs can impact the admission and impact the eventual discharge.
Objectives:
- Recognize how mental health needs impact the medical admission
- Assess how mental health needs will impact disposition
- Design a disposition plan that considers health and mental health

Speaker: Dr. Mary Beth Their, DNP,CRNP Nurse Practitioner at UM Upper Chesapeake Medical Services
Abstract: Palliative care has shown rapid growth across the country in recent years with significant increase in hospitals reporting palliative care team, from 25% to 83% over 20 years which highlights the growing recognition of the value in palliative care teams, (Center to Advance Palliative Care, 2022). Palliative care is holistic care addressing the whole person including physical, emotional, social and practical needs of someone suffering from a serious or chronic illness. Ultimately, palliative care teams enhance the quality of life for patients and their families by addressing medical, emotional, social, and practical needs
Objectives:
- Define Palliative Care & Identify populations that benefit from palliative care
- Summarize the effect of Palliative Care has on patient outcomes and cost
- Discuss the inpatient vs outpatient role of Palliative Care

1:15PM – 2:15 PM: Human Trafficking: What Is It, How to Recognize Victims, & To Avoid Recidivism and Build Resilience

Abstract: The 2024 Trafficking in Persons Report published by the US Department of State acknowledges that 27 million people are trafficked globally. Human Beings are forced into work that includes labor, sex work, and service work. Others are migrants who seek a better life and wind up being forced to work in circumstances where they are abused, underpaid, and forced to do work they had not signed up to do. Once trafficked, it becomes difficult for victims to extricate themselves. This session will present and overview of the federal legislation, discuss grooming, identifying victims, mandatory reporting requirements, reasons for recidivism, and resilience among trafficking victims.
Objectives:
- • Explain the federal legislation behind human trafficking
- Describe mandatory reporting requirements as it relates to trafficking
- Discuss several resilience-building- strategies to help both victims of trafficking and clinicians working with trafficking victims cope with the stress of rebuilding lives.
