2010 ACMA Transitions of Care Conference

Mark November 10-11, 2010 on your calendar, and plan to join us for the 2010 ACMA Transitions of Care Conference at the Peabody Little Rock hotel in Little Rock, AR. Dr. Eric Coleman, noted author and leader in transitions of care initiatives such as the Care Transitions Program, will speak and serve as the event’s course director.

The conference is designed for Hospital/Health System Case Management professionals in leadership positions. The conference is a one-day intensive workshop focused on Transitions of Care. The objective of this year’s conference is to educate Case Management leaders on the transitions of care movement, what it means to the practice of Case Management, and how they can better prepare for the increased focus in this area.


Eric Coleman, MD, MPH
Featured Speaker & Course Director
Dr. Eric Coleman, MD, MPH, is a Professor of Medicine within the Divisions of Health Care Policy and Research and Geriatric Medicine at the University of Colorado Denver. Dr. Coleman is the Director of the Care Transitions Program, aimed at improving quality and safety during times of care “hand-offs”. He is also the Executive Director of the Practice Change Fellows Program, designed to build leadership capacity among health care professionals who are responsible for geriatric programs and service lines. As a board-certified geriatrician, Dr. Coleman maintains direct patient care responsibility for older adults in ambulatory, acute and sub-acute care settings.

Dr. Coleman's research focuses on: (1) enhancing the role of patients and caregivers in improving the quality of their care transitions across acute and post-acute settings; (2) measuring quality of care transitions from the perspective of patients and caregivers; and (3) implementing system-level quality improvement interventions and (4) using health information technology to promote safe and effective care transitions.



Cheryl Phillips, MD, AGSF
Faculty
Cheryl Phillips, MD, AGSF, is the Chief Medical Officer of On Lok Lifeways. She also serves on the NCQA Geriatric Measurement Advisory Panel. She has twice provided testimony to the U.S. Senate Special Committee on Aging, as well as testified to the House Appropriations Committee. In 2002, she served as one of 30 fellows for the Primary Health Care Policy Fellowship under Secretary Tommy Thompson, Department of Health and Human Services.

She has served as president of the American Medical Directors Association, president of the American Geriatrics Society (AGS), and is currently the chair of the Board of Directors for AGS. In 2005 she was appointed by Governor Schwarzenegger as a Governor’s delegate to the White House Conference on Aging, and was also appointed to the California Commission on Aging and the California Olmstead Committee, where she continues her service.



Carol Levine
Faculty
Carol Levine directs the United Hospital Fund's Families and Health Care Project, which focuses on developing partnerships between health care professionals and family caregivers, especially during transitions in health care settings - www.nextstepincare.org. Before joining the Fund in 1996, she directed the Citizens Commission on AIDS in New York City from 1987 to 1991, and The Orphan Project, which she founded, from1991 to 1996. As a senior staff associate of The Hastings Center, she edited the Hastings Center Report.

Ms. Levine is also an accomplished editor who has contributed her expertise to recent health care publications. In 1993, Ms. Levine was awarded a MacArthur Foundation Fellowship for her work in AIDS policy and ethics. She was named a WebMD Health Hero in 2007.



Kevin M. Terrell, DO, MS
Faculty
Kevin M. Terrell, DO, MS, is a board-certified emergency medicine physician and an Associate Professor of Emergency Medicine at Indiana University. He is Chair of the Department of Emergency Medicine Research Committee and a member of the Department’s Faculty Development Committee. Dr. Terrell is a Research Scientist in the Regenstrief Institute, Inc. and a Center Scientist in the Indiana University Center for Aging Research.

Dr. Terrell is a 2004 recipient of a Dennis W. Jahnigen Career Development Award, which is supported by the American Geriatrics Society, the John A. Hartford Foundation, and Atlantic Philanthropies, Inc. He was awarded a 5-year K23 Mentored Patient Oriented Career Development Award from the National Institute on Aging in 2008. Dr. Terrell has also received research funding from the Retirement Research Foundation, the Donald W. Reynolds Foundation, and the Indianapolis Retirement Home Fund.
