Welcome to the ACMA Massachusetts Chapter webpage! The MA Chapter has been designated as a Chapter of Merit Award, which is an award given to recognize outstanding chapter performance. ACMA Massachusetts Chapter/Greater Region Mission: To provide educational programming and networking opportunities to case management and transitions of care professionals across the healthcare continuum in Massachusetts, New Hampshire, Rhode Island, Maine, and Vermont. Case management professionals include nurses, social workers, physician leaders, and care transitions teams.

Conference Posters
Every year, we dedicate time and space at the annual ACMA MA Chapter conference for posters that showcase projects that have had a positive impact or have provided a valuable resource to your patients or facility. Please reference How-to create your poster if you are considering sharing your work in the future. Reasons for presenting a Poster include:
- Sharing valuable findings and organizational advances with your peers.
- Highlighting your successes and learn how others have overcome challenges.
- Building your resumé.
- Networking with your peers.
- Using this as a starting point for other opportunities (articles, podium presentations, etc.)
- Being recognized for Best Practice, Most Innovative Topic or Ability to Replicate/Implement.
