Represents Case Management nurse and social work professionals who reside or work in Illinois.

Yearly Chapter Dues: None 

The Chapter of Excellence and Chapter of Merit program defines outstanding chapter performance by outlining the standards of practice for optimal chapter operation. Chapters achieve these awards by demonstrating performance in the key areas of chapter operations and attaining specific growth and development benchmarks.

Presenter: Kristin Thrun (Administrator) and Mike Hensley, Burgess Square Healthcare and Rehab Centre. Burgess Square is a 5-star CMS rated skilled nursing facility located in Westmont, Illinois.

PDPM goes into effect October 1, 2019

Attendees will learn:

  • Understand the basic framework of the PDPM model.
  • Learn changes in documentation requirements for SNFs based on PDPM model.
  • Demonstrate understanding of the key elements for the PDPM program and how it will impact the Skilled Nursing Facility profession.

Presenter: Pete Miska is the Owner/Administrator of Phoenix Home Care, L.L.C.  Pete is a thirty-year veteran in the post-acute care continuum. This includes the Home Health, Medical, Infusion and Hospice industry. He currently is the president of Phoenix Home Care, L.L.C. Phoenix Home Care is an IDPH deficiency free, Joint Commission Accredited, 5-star CMS rated Medicare Certified Home Health Agency that has been servicing Illinois families for over 20 years.

Attendees will learn: 
     - 2019 Changes coming to Medicare Home Health Benefits.
     - Medicaid changes and reimbursements. 
     - PDGM/ Value based/ Bundle Payments 
     - IMACT ACT- How results are measured and what is in store for the future.
