ACMA Massachusetts Chapter Mission: To provide educational programming and networking opportunities to case management and transitions of care professionals across the healthcare continuum in Massachusetts and its surrounding region (New Hampshire and Maine CMs are welcome!). Case management professionals include nurses, social workers, physician leaders, and care transitions teams.
Annual Meeting
