Join us for this one day of education and networking for the 3rd Annual New Jersey Chapter Case Management Conference of the American Case Management Association. 

After Attending this Conference, you will be able to:

  • Examine strategies and expectations to engage nurse and social work case managers, physicians and interdisciplinary team members to affect health care change
  • Identify and apply care coordination strategies that successfully prevent unplanned readmissions
  • Apply case management tools, resources and approaches to promote patient engagement and self-management
  • Apply clinical judgement and documentation review to validate medical necessity, appropriateness of level of care and resource utilization
  • Provide appropriate care and counsel and their families

NJ Social Workers: An application has been made for consideration of 5 general CE credits for this conference. CE certificate will be emailed within 10 days to social workers who sign in and out of each session attended and complete session evaluations—including an overall conference evaluation. The evaluation forms will be distributed upon check-in of each session and should be turned in to staff members by end of day on June 22, 2019.
