Your Northern California Chapter (est. 2003)

We proudly represent Nursing and Social Work Case Management professionals who reside or work in Santa Cruz, Santa Clara, Stanislaus, Calaveras, Amador, San Joaquin, Alameda, Contra Costa, San Mateo, San Francisco, Marin, Solano, Sacramento, El Dorado, Placer, Sutter, Yolo, Napa, Sonoma, Lake, Colusa, Yuba, Nevada, Sierra, Plumas, Butte, Glenn, and Tehama counties.

Case Managers and Social Workers in general work along side one another through the continuum of health care. We want to Thank You for all your work with patients and the frequently complex interactions involved in patient care. Please take time to honor the work of your fellow social workers as you go through your day today. We are in especially trying times in much of Northern California with COVID 19 and are called upon to be front line in the face of a mounting public health crisis. Please remember to stay safe and practice self-care through these stressful times.

Special Congratulations to the Northern California Chapter for ELEVEN consecutive years of earning Chapter of Excellence (COE). The Chapter of Excellence designation defines outstanding chapter performance by outlining the standards of practice for optimal chapter operation. Chapters achieve these awards by demonstrating performance in the key areas of chapter operations and attaining specific growth and development benchmarks.

Annual Meeting

After careful evaluation and consideration of the COVID-19 pandemic, the ACMA Northern California Chapter Board has made the decision to shift our 17th Annual Case Management and Transitions of Care Conference to a virtual format.

Public health is our highest priority and we understand how critical you are to your organizations and the communities you serve. We will be offering a virtual option for all attendees that will include the opportunity to access the same educational content, earn CE credit, learn about the latest innovations in case management from our exhibitors and share knowledge with your peers. Your current registration will be automatically transferred to the virtual conference.

We will host one live session on July 3, with additional on-demand sessions offered shortly after. Specific agenda and detailed virtual conference information will be sent via email as soon as they are confirmed. We ask for your patience as we finalize these details.

Please note that you are responsible for canceling or modifying any hotel and travel arrangements directly.  

Thank you for continuing to support the case management profession and we look forward to creating a new experience for our attendees.

The Accredited Case Manager (ACM) Certification is for health care delivery system case management professionals and tests core case management knowledge that is shared by nurse and social work case managers. Case Management Administrator Certification (CMAC) validates the competency of case management administrators, managers and aspiring leaders and promotes professional practice standards. If you are not yet certified, we encourage you to apply and enhance your professional practice by putting a credential after your name.
