Chapter Mission:

To provide educational and networking opportunities to Health Care Delivery System Case Management and Transitions of Care Professionals in the State of Connecticut and surrounding states.

CHAPTER OF EXCELLENCE
The Chapter of Excellence program defines outstanding chapter performance by outlining the standards of practice for optimal chapter operation. Chapters achieve these awards by demonstrating performance in the key areas of chapter operations and attaining specific growth and development benchmarks.

2018 marks the fourth consecutive year that the CT Chapter has achieved Chapter of Excellence!  

As stated by Nancy Jubinville, "The CT Chapter is very lucky to have an incredibly talented and dedicated Board of Directors and committee members. They strive to achieve goals of the chapter and to best serve members. The #1 focus is always about CT chapter members--how to support them in their profession, keep them updated with education/networking sessions and assist them in achieving the ACM certification. Satisfied, engaged members equate to chapter excellence."

Senate Passes Opioid Legislation in a 99-1 Vote

The opioid legislative package passed by the Senate in a 99-1 vote on Monday, September 17, creates $8 billion in funding. Difficult negotiations are ahead with the House of Representatives and the White House over key provisions that were not included in the Senate package. Read more here.

Bipartisan Opioid Package Includes Over 70 Senate Proposals

Since 2015, Congress has been intensely focused on addressing the nation’s opioid addiction epidemic. To date, it's efforts have already produced two landmark pieces of legislation. On Monday, the Senate is expected to pass the Opioid Crisis Response Act of 2018—a bipartisan package of over 70 proposals recommended by five different committees. Read the full story here.

Present your information in an informal yet professional forum at the 2019 ACMA National Case Management and Transitions of Care Conference, April 14–17, in Seattle. We're always looking for new and innovative ways to improve the practice of case management. If you have a program or project that has had a positive outcome or provided a valuable resource to your patients, facility or agency, we encourage you to submit your poster presentation by September 21.
