Welcome to the ACMA Minnesota Chapter! (est. 2010)
Represents Case Management professionals, Registered Nurses, Social Workers, Physician Advisors, Case Management Assistants, Nurse Practitioners, and Care Transitions staff, who reside or work in Minnesota and/or neighboring states without a local chapter.

The event will be an opportunity to share knowledge and learn with case management and transitions of care professionals in health care. Take advantage of this unique professional development event right in our backyard and improve patient outcomes.

Greetings MN ACMA !!
Gratitude and Appreciation for all that attended our Second First Annual Fall Conference 2026.
We had over 75 attendees, great content and a lovely MN fall day!
We had 12 Exhibitors, and a huge thank you to each one of them for supporting our Chapter Conference and sharing their services with our attendees.
A big thank you to our Conference site host - The Commons of Marice, the atrium was a beautiful place to learn, network, and meet up with friends or make new friends.
The last thank you goes to the Education and Exhibitor Committees for pulling together fabulous speakers, variety of topics, and great interactions with the exhibitors.
