Welcome to the ACMA Minnesota Chapter! (est. 2010)
Represents Case Management professionals, Registered Nurses, Social Workers, Physician Advisors, Case Management Assistants, Nurse Practitioners, and Care Transitions staff, who reside or work in Minnesota and/or neighboring states without a local chapter.

Save the date for the 16th Annual ACMA Minnesota Chapter Conference to be held May 8, 2025 at Minneapolis Marriott Northwest.
The event will be an opportunity to share knowledge and learn with case management and transitions of care professionals in health care. Take advantage of this unique professional development event right in our backyard and improve patient outcomes.

The Minnesota Chapter of ACMA would like to thank all our attendees and vendors who supported our May 2nd Conference. We had a great day of learning from community experts, networking with peers, and making new friends.
Please save the date for 2025: May 8th, 2025 (Thursday) and we will be at the same location that were in 2024.
