Chapter Mission:

To provide educational and networking opportunities to Health Care Delivery System Case Management and Transitions of Care Professionals in the State of Connecticut and surrounding states.

CHAPTER OF EXCELLENCE
The Chapter of Excellence program defines outstanding chapter performance by outlining the standards of practice for optimal chapter operation. Chapters achieve these awards by demonstrating performance in the key areas of chapter operations and attaining specific growth and development benchmarks.

2018 marks the fourth consecutive year that the CT Chapter has achieved Chapter of Excellence!  

As stated by Nancy Jubinville, "The CT Chapter is very lucky to have an incredibly talented and dedicated Board of Directors and committee members. They strive to achieve goals of the chapter and to best serve members. The #1 focus is always about CT chapter members--how to support them in their profession, keep them updated with education/networking sessions and assist them in achieving the ACM certification. The 2018 Leadership and Physician Advisor Conference took place early November in Huntington Beach, California! Case management leaders and physician advisors from across the country gathered to collaborate, network and move case management forward. Congress Passes Opioid Bill, President to Sign

Great news from Washington! With your support, Congress took a historic step in the fight against the opioid epidemic.
