We represent Case Management professionals, RNs, Social Workers, Physician Advisors, Case Management Assistants, Nurse Practitioners, and Care Transitions staff who reside and/or work in the state of Georgia.

Thank you for another great Annual Conference!

Our 13th Annual Conference was held on Saturday, August 11th, 2018 at the Westin Buckhead Atlanta.

Plans are now underway for our Winter Case Management Conference - to submit ideas/comments/suggestions, please email us at acmagachapter@gmail.com

Save the date for our 14th Annual Case Management and Transitions of Care Conference, which will be held on Saturday, August 19th, 2019.  More information will follow.

We're always looking for new and innovative ways to improve the practice of case management. If you have a program or project that has had a positive outcome or provided a valuable resource to your patients, facility or agency, we encourage you to submit your poster presentation by September 21.
