We represent Case Management professionals, RNs, Social Workers, Physician Advisors, Case Management Assistants, Nurse Practitioners, and Care Transitions staff who reside and/or work in the state of New York.

Annual Meeting

Join your peers at the 6th Annual Case Management and Transitions of Care Conference for the New York Chapter on September 23, 2022, at the Long Island Marriott in Uniondale. 

The event will be an opportunity to share knowledge and learn with case management and transitions of care professionals in health care. Take advantage of this unique professional development event right in our backyard and improve patient outcomes. 

ACMA Announcements
The 2022 Leadership and Physician Advisor Conference (LPAC) will be an opportunity to share knowledge and learn with physicians and case management leadership. Gain new strategies when you join your peers from across the country to discuss advancing patient care. Prepare for the ACM exam with the Summer ACM Workshop. The five-hour course, broken into two, 2 1/2 hour sessions, will begin at 9:00 a.m. CT on both Saturday, July 23 and Saturday, July 30. Registration closes on Wednesday, July 20. Learn more and register >>
