Chapter Mission:
To provide educational and networking opportunities to Health Care Delivery System Case Management and Transitions of Care Professionals in the State of Maryland and surrounding states.
Case Management professionals include RN's, Social Workers, Physician Advisors, Nurse Practitioners, Care Transitions team members, Case Management Assistants
Chapter Dues: Free with National ACMA membership

The Chapter of Excellence program defines outstanding chapter performance by outlining the standards of practice for optimal chapter operation. Chapters achieve these awards by demonstrating performance in the key areas of chapter operations and attaining specific growth and development benchmarks.

Annual Meeting
Join us for our 18th Annual ACMA Maryland Chapter Case Management and Transitions of Care Conference
Public health is our highest priority and we understand how critical you are to your organizations and the communities you serve. We are doing things a little differently this year in order to address the needs of everyone who attend our conference.
We're excited to bring the 18th Annual ACMA Maryland Chapter Conference to you virtually this year, with LIVE sessions taking place on September 12, 2020 for all attendees which includes the opportunity to access the same educational content, earn CE credit, learn about the latest innovations in case management from our vendor community partners and share knowledge with your peers with networking capabilities from your home.
If you join us during the LIVE conference on September 12th, you will earn 5.0 CE’s BUT you still have the opportunity to earn 3.0 more CE’s because you can access the other breakout sessions that you didn’t choose on the LIVE conference day up to 60 days after the live conference! So 8 CE credits in all!!
Another option: The sessions will be recorded and available to you via our virtual conference platform for 60 days after the September 12th conference for full 8 CE credits.
****Note: Virtual LIVE sessions are a requirement for Social Work CE’s in Maryland.
Those who have already registered, will be automatically transferred to this virtual conference.
