  1. Describe lessons learned during the pandemic for VA services.
  2. List common barriers and solutions to overcome roadblocks.
  3. Identify key resources to facilitate care coordination within the VA.

 Nursing: This CE activity is approved by the American Case Management Association (ACMA) for 1 contact hour (based on 50 & 60-minute hour).

ACM / CMAC: Approved by the American Case Management Association (ACMA) for 1 ACM & CMAC Recertification credit.

CCM®: Through a collaborative agreement with the Commission for Case Manager Certification (CCMC) this activity and the corresponding credits are acceptable for CCM® renewal for 1 CE.
