As providers, we work with patients and families from birth until death. Often the end-of-life process and the time leading up to this can be difficult to navigate. This discussion will provide health care professionals with tangible takeaways such as a structured style called the Serious Illness Conversation as well as what to do after patients and families determine their wishes for care. Education will be provided on documents that can be completed to help ensure wishes are followed through upon entry into the healthcare system.

Kate is a certified Advanced Hospice and Palliative Care Social Worker. She has worked in the health care setting for the past 15 years with 13 of those years specializing in the Palliative Care field in the acute setting. Currently, Kate is transitioning to the Ambulatory Palliative Care field.
