About National Case Management Week
National Case Management Week 2021 (NCMW2021) is October 10-16, 2021! Join us as we recognize and show appreciation for case management and transitions of care professionals across the country. This year's ACMA theme will be announced later this summer. ACMA hopes you will celebrate NCMW 2021 and recognize the dedication, compassion and patient care outcomes achieved by case managers in your organization. Across all care settings, case managers are making the difference!
2020 was different. Health care professionals from across the country were, and continue to be, challenged by a multitude of obstacles. Your efforts were appreciated as evidenced by the celebrations we saw through our social media feeds and local news affiliates. With each challenge we have heard countless stories of case management professionals stepping up, not back, to positively impact patients across the care continuum. Thank you for your efforts, your expertise, your dedication and your compassion.

Kris Nelson, Nurse Case Manager, Henry Mayo Newhall Hospital
Kris has worked tirelessly as a front line Nurse Case Manager on our COVID Unit since March of 2020. She is an exceptional patient advocate. She has been a highly visible and courageous presence on the Unit each day interacting with Nurses, Doctors.
Mark Denno
Mark coordinated dialysis care after patients were evacuated from the fires in Northern California to multiple locations in the state. This involved a lot of work to find dialysis centers and patients who were displaced to shelters or other family.

In conjunction with NCMW 2021, Wednesday, October 13 is ACM Recognition Day! This is the perfect opportunity to recognize the efforts of those case management professionals who have earned their Accredited Case Manager (ACM) certification and those who are in the process of becoming ACM certified. Many organizations will choose this day to celebrate ACM professionals with a special event, congratulations or gift to recognize this important industry distinction.
