SESSION ABSTRACT: Children’s Hospital Los Angeles (CHLA) is a high-acuity, high-volume academic medical center that treats the most vulnerable children of Southern California and beyond. With increasing demand, capacity constraints and resource limitations, the organization needed to revamp its inpatient operations to meet these challenges. Review the varying models of hospital reimbursement and how each model can be affected by different interventions, such as length-of-stay reduction, clinical documentation improvement, physician advisor program development, avoidable delay tracking and other tactics which led to significant operational improvements at CHLA. Learn how these interventions can help your hospital improve inpatient access, throughput and facility reimbursement.

Session 4 Understanding the Social Determinates of Health and Their Impact on Length of Stay

SESSION ABSTRACT: The case manager’s ability to understand the complex psychosocial factors impacting every patient’s unique health experience is critical, particularly with patients at risk for a long length of stay in the hospital. These patients can become deprioritized on the case manager’s caseload because of the perceived difficulty and multiple barriers to transitioning smoothly and safely back to the community. Review successful strategies utilized by a large teaching hospital in California to reduce the LOS for patients in the hospital more than 30 days.

SESSION ABSTRACT: The stream of new Medicare regulations never seems to stop running. 2018 brought us the removal of total knee replacement from the inpatient-only list and discussion about moving many cardiac procedures to surgery centers, the Medicare Advantage plans seem to continue to test our patience and the 2019 CMS on the inpatient admission order created a new spin. Hospitals are also faced with the ever-increasing numbers of Medicare Advantage patients. This session will review the most recent regulatory updates that case managers need to know.
