Case Management Readiness for Longitudinal, Value-Based Care

Publication Date: 3/31/2020

Speaker: Debra McElroy, MPH, RN and Kathleen Ferket
Length:1 hour

Abstract Transitions of care between care settings or levels of care have historically focused on the episode of care, with the case manager focused on the immediate transfer of information to a discrete point on the care continuum. As healthcare systems aspire to and move toward value-based care, today’s dynamic environment requires a comprehensive approach to care coordination. This session will focus on a discussion of findings from the ACMA 2019 National Survey and the ACMA Transitions of Care National Data Set and will also provide strategies for preparing the case manager of the future.

Learning Objectives

1. Describe data findings from the ACMA 2019 National Survey and explain how it impacts future considerations for case management.

2. Explain and interpret trends found in the ACMA Transitions of Care National Data Set, obtained from organizations performing a self-assessment on the Transitions of Care Standards.

3. Discuss strategies and tools to prepare case managers for practice in today’s dynamic healthcare environment.
