Tips and Tools to Manage Skilled Facility Transitions

This presentation provides an overview of reasons for re-admissions, comparative benchmarking and common interventions. Strategies to develop a “safety net” to address uncontrolled advancement of chronic disease during transitions to home or to skilled facilities are discussed. Communication and the presence of goals for care discussions play a key role. A step by step tool kit is shared to increase awareness of palliative care services, implement goals of care discussions and track outcomes.

Note: This presentation includes ethics/decision making content related to patient empowerment and goals of care program implementation.
