Transitions of Care: A Firm Handshake With Our Outpatient Case Managers

In the changing world of health care, the responsibilities and duties are expanding for the case manager. New models of case management emerge because of new initiatives the federal government has put forth in the last four years. The reduction in reimbursement in Medicare payments for re-admissions and the new modes of payment make the case manager an essential person on the clinical team that can provide a safe, timely, appropriate, and sustainable discharge. We therefore find increasing numbers of case managers in the outpatient areas. These include nurse navigators, chronic care coordinators, insurance complex case managers, as well as clinic care coordinators imbedded in the outpatient clinic working closely with a physician. Close and continuous communication between an inpatient case manager and an outpatient care coordinator can create the type of discharge that enables the patient to stay in the community, in the home, and using health care resources that are less costly. Care moves away from acute care to preventive care and the primary care physician becomes the driver of complete and coordinated care.
