Addressing Challenges for Patients with Substance Use Disorders Who Require Outpatient Parenteral Antibiotic Therapy

Abstract
Have you considered the challenge involved in arranging outpatient parenteral antibiotic therapy (OPAT) for patients with a history of substance use disorder (SUD)? At a community hospital south of Boston, a group of nurses expressed moral distress and questioned best practice for this challenging question. A conversation between two nurses was the catalyst to practice change. This presentation will review the steps involved in completing a literature appraisal, engaging the interdisciplinary team, collecting baseline data and creating a pilot.

1. Describe evidenced-based project to provide home IV antibiotic therapy to patients with substance use disorders.
2. Explain how collaboration can lead to practice change.
3. Utilize case studies in order to evaluate current process and opportunity for practice change.


Speaker
Erika Cahill, BSN, RN, ACM-RN, RN Case Manager, South Shore Hospital
Erika Cahill is a nurse case manager in the Department of Care Progression at South Shore Health System. She has a BS in Nursing, a BS in Humanities and holds an ACM (Accredited Case Manager). Erika has 10 years of nursing experience, which includes direct patient care in the acute setting, case management in home care, and case management inpatient setting with a focus in orthopedics and trauma patients. Erika currently works in the emergency department at a busy community hospital south of Boston. Her role as a case manager allows her to pursue her passion for community advocacy in addition to assisting the hospital with patient flow, discharge planning and utilization review.
