Coronavirus: The Impact on Case Management and Transitions of Care

Abstract
The American Case Management Association convened healthcare professionals and a legislative expert to provide our ACMA community on how case management and transitions of care professionals can support patients, communicate with the health care community and be a resource to other providers during the COVID-19 outbreak. Our health care leaders and legislative expert will discuss discharge and transition strategies, redeployment of clinical services, legislative update—key bill provisions and regulatory changes affecting the practice, resources to educate you, your staff and patient. COVID-19 has turned our daily routines, lives, and businesses upside down. Many of you have been pulled into different roles, filling gaps where your hospital systems and your communities need you. Others have remained in their current departments, dealing with higher caseloads with fewer case managers. Others are busy interpreting and implementing new and changing guidelines from federal, state, and local levels. This session will share how healthcare professionals are dealing with the impact of the Coronavirus and resources you can utilize at your organization.

1. Describe the impact of the Coronavirus on case management and transitions of care.
2. Review legislative updates.
3. Identify lessons learned related to redeployment and discharge transition strategies.
4. Recognize resources available to you, your patients, and your organization.


Speakers
Angie Roberson, MSN, RN, ACM-RN, Director, Case Management, Spartanburg Regional Healthcare System
Angie Roberson is a nurse with over 25 years of experience in healthcare and 19 years in various case management roles. She earned her bachelor's degree from the University of South Carolina Update and her master's from Gardner Webb University. She resides in South Carolina and has worked for Spartanburg Regional Healthcare System for 19 years; currently working as Director of Case Management for 5 acute care facilities. Angie has been a leader in Case Management serving at both the state and national level and currently serves as the President of The American Case Management Association (ACMA). She works closely with her Medical Director facilitating an internal physician advisor team. In her current role she serves on the Utilization Management committee, Executive Compliance Committee, Revenue Cycle Integrity committee, and the Quality Process Improvement Council and has responsibility for strategic initiatives around avoidable readmissions and care transitions.

Max Perkins, BA, Senior Vice President, Lobbyit.com
Max Perkins is the Senior Vice President at Lobbyit.com and is the project leader for ACMA's government affairs team. He began his career in Washington on Capitol Hill in 2008 as a non-partisan staffer for the U.S. Senate Committee on Rules and Administration. Max moved away from the Hill in 2010, taking a position as a legislative assistant at a top five lobbying firm in Washington. He left that firm in 2017 to become Vice President at Lobbyit. Max has been working with ACMA since his arrival at Lobbyit.

Stephen Crouch, MD, Medical Director, Advocate Aurora Health
Dr. Crouch is the Medical Director for Integrated Care Management at Advocate Aurora Health located in Downers Grove, IL. He has the accountability for Physician Advisor, Utilization Management, and Clinical Documentation Improvement activities for Illinois hospitals within Advocate Aurora Health. His role is critical to ensure physician support across the enterprise and to provide guidance to attending physicians so they can make informed decisions regarding the appropriateness of hospitalization, continued stay and use of resources for patients. Further, he serves as a resource regarding state and federal utilization and quality regulations. Dr. Crouch received his medical degree from the University of Tennessee Health Science Center and completed his residency in emergency medicine at Carolinas Medical Center in Charlotte, NC. Dr. Crouch is board certified in emergency medicine and continues to work in the emergency department at Advocate Good Samaritan Hospital.

Marlene Bober, Senior Vice President of Client Services and Professional Practice, American Case Management Association
Marlene Bober, MHA, ACM - RN is the Vice President of Practice Development at the American Case Management Association. She is responsible for influencing initiatives and services that define and impact the practice of Case Management. This includes Standards of Practice, conference content programming, certification services, ACMA’s Advanced Care Transitions Simulation (ACTS) training program and the Association of Physician Leadership in Care Management (APLCM). With over 20+ years of leadership experience in the healthcare industry, she is recognized for her expertise in the areas of Population Health, Nursing Operations, Quality, and Care Management and Clinical Documentation Improvement. Marlene received her Masters of Science Degree in Healthcare Administration. She has successfully completed the Project Management in Health Care Certificate Program at Harvard School of Public Health and hold the ACM Certification. Marlene has published and presented nationally.
