The Role of the Physician in Transitions to Post-Acute Rehabilitation

Abstract
The PM&R physician plays a key role in the transition from acute to post-acute rehabilitation settings. With CMS changes beginning October 1, 2019 the role of the ?physiatrist will become even more important in determining the appropriate level of post-acute rehabilitation care. The CARE tool items initiated by CMS in October will impact the functional assessments of patients, and in turn will impact the reimbursement for the post-acute rehabilitation setting. The physician’s skilled facilitation of care will not only ensure the appropriate level of care, but also that the appropriate reimbursement is received for the post-acute rehabilitation episode.


Learning Objectives:
1. Describe the CARE tool and its impact on reimbursement for rehabilitation facilities.
2. Explain the role of the medical director/physician advisor in the post-acute rehabilitation setting.
3. List practical recommendations on working with PM&R physicians to ensure smooth transitions to rehabilitation settings.


Speakers
Dr. Anjum Sayyad, MD, MBA
Dr. Anjum Sayyad received her MD from Chicago Medical School. She completed her internship in Internal Medicine at Advocate Lutheran General Hospital and went onto residency training at the Rehabilitation Institute of Chicago, part of Northwestern University. She has completed double board certification in physical medicine and rehabilitation, and brain injury medicine. She has served as brain injury medical director (2013-2016) and residency program director (2016-2019) for Marianjoy Rehabilitation Hospital. She now currently serves as chief medical officer of Chestnut Health, a physiatrist group of 20 providers that services 30 Long term acute care hospital and skilled nursing facilities, across five states.
