Abstract
Join in on a discussion of different techniques, innovations and opportunities to incentivize and reward your eco-system of health care providers around you and your organization. Leaders from two major health systems will share their past journeys to transform their relationships with community partners and discuss current challenges and opportunities.

Speaker:Marcia Colone, PhD, MS, LCSW, ACM-SW | Vice President, Care Coordination and Transition Management | Vanderbilt University Medical Centern

Dr. Marcia Colone is the Vice President of Care Coordination and Transition Management at Vanderbilt University Medical Center. Prior to joining Vanderbilt, she was the System Director, Care Coordination, Clinical Social Work and Patient Progression at UCLA Health System and the Director, Care Coordination, Bed Placement, Pastoral Care, Social Work at Northwestern Memorial Hospital. Dr. Colone graduated with a Ph.D. in social welfare administration from the University of Chicago.

Speaker:Russel Graney, BA | Founder and CEO | Aidin

Russ Graney began his career at Bain and Company in New York working with Fortune 50 companies in tech, finance, and consumer products. In 2011, Russ led the sale of his private equity firm’s largest investment, a case management services company. Russ left PE to build Aidin when his uncle was diagnosed with early on-set Alzheimer’s. He is originally from Fairfax, VA and graduated magna cum laude from Harvard College.
