Abstract
Partnering with community-based organizations (CBO) and using evidence-based social models is invaluable in helping to reduce avoidable and unnecessary readmissions. Establishing collaborative relationships reduces duplication and promotes efficiency while providing patients with an extended level of care at home, resulting in enhanced satisfaction and overall patient health. Panelists in this session will share best practices, tools and essential information to address SDoH and develop effective partnerships with community organizations.

Learning Objectives
1. Identify at least two key elements to consider when partnering with community-based organizations.
2. Compare social based care transition programs and potential outcomes with different approaches.
3. Apply new concepts to implement social based care transition programs that target individuals at risk for potential readmission.

Speaker: Patricia "Pat" Blaisdell | FACHE Vice President, Policy | California Hospital Association

Patricia Blaisdell is Vice President, Continuum of Care for the California Hospital Association. Ms. Blaisdell provides membership support and advocacy for hospital-based post-acute care and case management services, including policy analysis and interpretation and communication with legislative regulatory bodies. Ms. Blaisdell has over 25 years of experience in hospital and health care management in acute and post-acute settings. She began her career as a speech/language pathologist: her work was recognized with the Outstanding Clinical Achievement Award from the American Speech-Language-Hearing Foundation. Ms. Blaisdell, who is Fellow with the American College of Health Care Executives, has also been recognized with the ACHE Regent’s Award, and the Rural Champion in community leadership award from the California Rural Health Care Association.

Speaker: Gilbert Dorado, LCSW, ACM-SW | Regional Director, Care Management Operations | Providence St. Joseph Health

Gilbert Dorado is an accomplished Licensed Clinical Social Worker who has over 15 years of experience in case management including acute medical, behavioral health, primary care, and complex care management. Gilbert is currently serving as the Regional Director of Care Management Operations for Providence St. Joseph Health in the Southern California Region. He is responsible for the care management operations within 10 hospitals, a transitional care management team focusing on Medicaid patients and a team of community health workers working to address Health Equity within Los Angeles County. Gilbert is an Accredited Case Manager through ACMA, earned a bachelor’s degree in Community Health from New Mexico State University and a Master of Social Work from Our Lady of the Lake University. Gilbert has been an active ACMA member as he served on the ACMA National Board, co-chairs the National Fundraising Committee and has served many roles within the Southern California ACMA Chapter.

Speaker: June Simmons, MSW | President/CEO | Partners in Care Foundation

June Simmons has pioneered the development and scaling of evidence-based innovative interventions for the management of medications at home, self-management of chronic conditions, coordinated care to improve health outcomes, and care transitions. Throughout her distinguished career, she has been instrumental in envisioning, creating, funding and operating forward-looking health and social service programs that meet the mutual needs of patient populations, health care delivery networks and health plans. Her priority is sustainable patient-driven integration of care across settings, from primary care and hospital to home and community in the 21st century. June serves on the board of the National Council on Aging and has been honored by numerous organizations, including the California Social Work Hall of Distinction as the Los Angeles Aging Advocacy Coalition 2019 Champion and Redefining American Healthcare Award by the Healthcare Leadership Council in 2020. In 2021, she has been named Co-Chair of the National Aligning Social Care Workgroup and to the Advisory Board of the Coalition for Compassionate Care of California. She is a member of the Executive Roundtable, Cynosure Health Executive Leadership.
