Scaling Social Determinants of Health Screening Across the Care Continuum

Abstract
To enable a leading statewide clinical service provider to leverage emerging evidence of and maximize impact on Social Determinants of Health (SDoH), this provider made SDoH a key clinical, administrative and community-based priority. By creating a uniform and universal process to collect SDoH data across the care continuum, this provider informs hospital Community Health Needs Assessments, catalyzes the development of new programs and continued expansion of existing programs, engages payers to reimburse for SDoH programming, and most importantly, helps to identify and connect more high-risk patients to needed service. You will gain a foundational view of a system-level implementation plan for uniform screening of SDoH across the care continuum within a large health system. Additionally, you will gain insight into the use of data analytics and tools to drive both operational success and strategic planning as it relates to SDoH screening and intervention.

Colleen Sullivan earned a BA in Sociology from George Washington University, an MSW from The University of Pennsylvania, and an MBA in Health Care Management from The University of Texas at Tyler. She currently leads care management strategy, innovation and operations for a large integrated health care system, bridging ambulatory, acute and post-acute settings and teams.

Karen Vanaskie Scholarship Fund

Karen Vanaskie was one of case management's brightest and most dedicated professionals. She was serving as Secretary / Treasurer on the ACMA National Board of Directors at the time of her unexpected passing on June 21, 2024.

At the request of Karen's family, ACMA has set up the Karen Vanaskie Scholarship Fund. Karen was passionate about the need for case management certification. She actively supported her staff in pursuing the Accredited Case Manager (ACM) Certification. To continue her legacy, this scholarship fund will cover the cost of their ACM certification application fee for those to whom it is awarded. We believe this continues Karen's legacy.
