What “Hospital at Home” Means for Payers, Hospitals and Providers

Abstract
Learn how to measure the return on investment (ROI) of payor-based case management and demonstrate the impact it can have through an internal and contracting lens. Identify how to capture case management activities that impact social determinants of health in your membership and demonstrate reductions in inpatient admissions, emergency room visits, and total medical spend.

1. Discuss how to measure mitigation of social determinants of health that reduce inpatient and outpatient spending.
2. Describe strategies that produce quantifiable social determinant outcomes.
3. Identify opportunities to reduce total medical spending.

Renee Bremer, MSN, RN, ACM-RN, CENP | Associate Director, Care Management | University of Michigan

Renee Bremer recently completed her DNP in Executive Leadership and is the Associate Director for Care Management at the University of Michigan Hospital, Michigan Medicine, Ann Arbor, Michigan. She has focused her quality improvement projects on discharge checklists, readmissions, and electronic handoff tools across the continuum of care, and most recently hospital care at home.

Jacquelyn Redding, RN, BSN, CCM | Director, Medicare Advantage Inpatient Utilization | Blue Cross Blue Shield Michigan/Blue Care Network

Jacquelyn Redding has a Bachelor of Science in Nursing from Lourdes University in Sylvania, Ohio and is a Certified Case Manager (CCM). She began her career working as a RN Community Health nurse with the National Heart Attack Risk Study, while concurrently working as a staff RN in a Medical/Surgical/Cardiac Stepdown Unit at St. Vincent Mercy Medical Center in Toledo Ohio. She has held various clinical and leadership roles in the health care system for over 27 years and currently serves as Director of Medicare Advantage Inpatient Hospital Utilization for Blue Cross Blue Shield of Michigan. Prior to her role as Director, Jacquelyn worked as manager of Blue Cross Blue Shield in Medical Affairs managing multiple teams to develop and implement organizational medical, benefit and payment policies. Prior to Medical Affairs she worked as a manager for Blue Cross Complete Medicaid Integrated Care Management managing multiple teams to deliver a comprehensive Care Management and Utilization Management Program for Michigan Medicaid membership.

Karen Vanaskie Scholarship Fund

Karen Vanaskie was one of case management's brightest and most dedicated professionals. She was serving as Secretary / Treasurer on the ACMA National Board of Directors at the time of her unexpected passing on June 21, 2024.

At the request of Karen's family, ACMA has set up the Karen Vanaskie Scholarship Fund. Karen was passionate about the need for case management certification. She actively supported her staff in pursuing the Accredited Case Manager (ACM) Certification. To continue her legacy, this scholarship fund will cover the cost of their ACM certification application fee for those to whom it is awarded. We believe this continues Karen's legacy.
