Abstract
As healthcare shifts to value-based care, effective post-acute management is essential to improving outcomes and reducing costs. This session explores how health systems can leverage post-acute analytics and technology-enabled workflows to enhance care coordination, optimize discharge planning, and prevent avoidable readmissions.

Attendees will learn how data-driven insights and predictive tools help case managers identify high-risk patients, select appropriate post-acute care settings, and standardize transitions of care. The session will also highlight how technology can streamline workflows, improve visibility into post-acute performance, and strengthen collaboration across the care continuum. Participants will leave with practical strategies to reduce length of stay, lower total cost of care, and succeed in value-based care models.

Learning Objectives
1. Recognize the value of data driven organizational assessment to identify key opportunity areas.
2. Standardize workflows and create strong technology-enabled solutions resulting in reduced hospital ALOS, emergency room visits & hospital re-admissions.
3. Identify high performing post-acute care partners.
4. Recognize the value of efficient care management execution to help hospitals succeed in today’s environment.

Deborah Karn Calvy, RN, BSN, MHA | Senior Advisor, Clinical Strategy | Repisodic, A Trella Health Solution

Deborah Karn Calvy earned her Bachelor of Science in Nursing from East Stroudsburg University and her Master of Science in Healthcare Administration from University of St. Francis. She is a licensed registered nurse and healthcare executive leader. She currently serves as Senior Advisor of Clinical Strategy at Repisodic, A Trella Health Solution, partnering with health systems and post-acute providers to strengthen care transitions, advance value-based care, and improve referral management and network performance through data-driven solutions. Previously, Calvy held senior leadership roles including Vice President of Care Management and Utilization Management, Director of Clinical Resource Management, and Population Health Area Director of Case Management and Social Work. She is recognized for her strategic, results-driven leadership in clinical operations improvement, population health integration, and value-based care optimization.

Ryan Miller, MA, MBA | Vice President | Repisodic, A Trella Health Solution

Ryan Miller earned a Bachelor of Arts in Political Economy from the University of California, Berkeley, a Master of Arts in International Economics from Johns Hopkins University, and a Master of Business Administration from Dartmouth College’s Tuck School of Business. He is Vice President and Co-Founder of Repisodic, A Trella Health Solution, a healthcare technology company he helped launch eight years ago, where he leads strategy, operations, and business development. With more than 18 years of experience across government and private sectors and over a decade in healthcare, Miller has focused on improving care transitions through technology-enabled solutions. Previously, he served as Director of Strategy and Policy at Siemens Healthcare in Washington, D.C., leading transformation and reimbursement strategies during the implementation of the Affordable Care Act. He is recognized for building scalable healthcare businesses and advancing patient-centered innovation.
