Healthcare reform challenges the way we think, deliver on, and ensure the safety, sustainability and engagement for those we seek to serve.  In response, professionals in case management must think, act and do different to ensure access, engagement, and cost-effective care.  We must think beyond existing boundaries and go where the patients need to us to go.  This session will provide one strategy and possible resource for organizations to survive and thrive under healthcare reform; moreover, improve upon and save the lives of our most high-risk and vulnerable.
  1. Identify three patient populations who could be targeted for interventions by a community case manager.
  2. Outline important skills, knowledge and abilities as well as basic functions of a community case manager.
  3. Identify critical and meaningful metrics to tracking successful interventions of a community case manager.
