Improving Transitions of Care & the COVID-19 Pandemic

Abstract
Transitioning your patient from acute care to post-acute care can be a manual, bogged down process. The transition of your patient, the process they go through and their experience along the way is something to focus on now, more than ever, in the new COVID-19 landscape we are living in. What happens when post-acute facilities lack the capacity and capability to safely treat patients? This presentation will reveal proven strategies on processes and systems to address LOS while maintaining quality care, patient safety and reducing readmissions while addressing recent struggles COVID-19 has played in post-acute care settings. Other ways transitions of care may be changing in the future which set the patient and provider up for a positive experience will also be explored.


Learning Objectives:
1. Recognize the challenges presented to transitions of care in the new COVID-19 landscape.
2. Identify ways providers can overcome newly presented challenges while still providing excellent patient care.
3. Evaluate steps that can be taken to improve transitions of care while reducing cost and readmissions and increasing positive patient outcomes.
