Care Transitions Today Podcast

Care Transitions Today is a podcast that focuses on healthcare case management and transitions of care throughout the healthcare systems. So much is happening in healthcare today, and consumers as well as healthcare professionals are required on a daily basis to respond and adapt. Case managers - those nurses, social workers, physicians and others - are at the heart of working with patients and families as they navigate the complexities of their care and assuring they have what they need to transition their care to home or where they find themselves in the "system.” Join host Deb McElroy in her discussions with experts in this space as they share their experience, wisdom, and expertise -- facing the challenges as well as embracing the opportunities of this unusual year and time. Whether you are new to case management or a seasoned leader, listeners are sure to take away new ideas to benefit themselves, their facility and their patients.

Meet Our Host

Debra McElroy joined ACMA as senior vice president for practice development in 2017. She oversaw association operations with a focus on national initiatives including transitions of care standards and measurement and the advanced care transition simulations (ACTS) program. She is the former national leader of nursing programs and services delivered to health care systems across the country. McElroy has extensive experience in the public health sector, previously directing health care coalitions and federally funded initiatives that established infrastructure for medical homes. She has a master’s degree in public health, a bachelor’s degree in nursing and has published on advanced practice providers, nurse retention, public health and transitions of care.

ACMA Announces Promotion of Marlene Bober to Senior Vice President

The American Case Management Association is pleased to announce the appointment of Marlene Bober, MHA, ACM-RN to Senior Vice President. Bober joined ACMA as Vice President of Practice Development in January of last year, leading ACMA’s certification services, oversight of the Association of Physician Leadership in Care Management (APLCM), the Transitions of Care Standards of Practice and served as a clinical resource for content development. Validate Your Expertise Through Professional Certification

The Accredited Case Manager (ACM) Certification is for health care delivery system case management professionals and tests core case management knowledge that is shared by nurse and social work case managers. Case Management Administrator Certification (CMAC) validates the competency of case management administrators, managers and aspiring leaders and promotes professional practice standards. If you are not yet certified, we encourage you to apply and enhance your professional practice by putting a credential after your name.
